* Visit the Montana DMV office * Complete and submit Release of Driving Records form
* Provide proof of identity such as ID card, passport or driver’s license
* Pay the $4 driving record fee
By mail
You can request your Montana driving record by mail by: * Completing and mailing notarized Release of Driving Records form along with a self-addressed, stamped envelope to: Motor Vehicle Division P.O. Box 201430 Helena, MT 59620
* Paying through check or money order $4 driving record fee
Online In order to make an online request for your driving request, visit the Montana DOJ website and:
* Choose your specific use statement
* Provide your name, driver’s license number and last four digits of social security number
* Pay through debit or credit card $ 7.25 online driving record fee
