* Visit the Montana DMV office * Complete and submit Release of Driving Records form

By mail

* Paying through check or money order $4 driving record fee
Online In order to make an online request for your driving request, visit the Montana DOJ website and:
* Choose your specific use statement
* Provide your name, driver’s license number and last four digits of social security number
* Pay through debit or credit card $ 7.25 online driving record fee
