All Iowa residents with a driver’s license can obtain a record of their driving history from Iowa’s Department of Transportation. If you make an Iowa Driving Records Request, the Department of Transportation will provide you a driving history report that contains:

- Fill out and submit Privacy Act Agreement for Request of Motor Vehicle Records(Form 431069)
- Submit your Iowa’s Driver’s license
- Pay in cash a $5.50 fee

- Mail a completed Privacy Act Agreement for Request of Motor Vehicle Records(Form 431069)
- Mail your Iowa’s Driver’s license
- Pay a $ 5.50 fee by check or pay order
Iowa Driving Records Online
Only you are allowed to request your Iowa driving records online. Others interested in accessing your driving records will have to make an Iowa Driving Records Request by mail or in-person. You can request make an online request for your Iowa driving records by:
- Visiting the MVD website
- Entering your name, DOB, last five digits of your Social Security Number (SSN) and driver’s license number
- Paying the $ 5.50 driving record fee plus an additional $ 1.50 service fee through your debit or credit card
